Notice on data collection consent and non-liability for medications: (CCHCS) may collect personal data to support a patient’s ability to speak with a Board-certified state medical provider over video through the website (email). (CCHCS) is not responsible for any misdiagnosis, and does not formally diagnose. (CCHCS) is not responsible for prescription errors, any complications from prescriptions, or any responsibility on improperly administering medications since self-administered by the patient and the patient is responsible for their own complications. (CCHCS) is not responsible for any injuries from the patient since medication is self-administered. (CCHCS) is not responsible for any medication prescribed incorrectly. For any complications patient should contact the pharmacy, or PCP or if medication is not working to their expectations.
How We Collect Your Personal Information
We may obtain Personal Information:

We may deny your correction request if, after considering the totality of the circumstances, we determine that the information is more likely accurate than not.
Non-Discrimination
Nevada consumers also have the right not to receive discriminatory treatment if they exercise the rights listed herein.
(CCHCS) cannot respond to your request or provide you with your personal information if (CCHCS) cannot verify your identity or authority to make the request and confirm the personal information relates to you. Making a verifiable consumer request does not require you to create an account with us. We will only use personal information provided in a verifiable consumer request to verify the requestor's identity or authority to make the request.
(CCHCS) conducts all medication requests through email and may require the patient to complete lab work, radiology, or see a specialist outside of CCHCS scope of practice. CCHCS may take up to 14 days to respond to patient regarding wellness recommendation. Patients will complete a medical questionnaire and assessment, may need to upload any recent labs or radiology reports or medical records for the medical team to review before making their recommendation.
We ask that you not provide us with any medical or protected health information through the above communications.
Additional Terms for EU Visitors
Children
This Service is neither directed to nor structured to attract children under the age of 13 years. Accordingly, we do not intend to collect Personal Information from anyone we know to be under 13 years of age. We will direct potential users under 13 years of age not to use the Service. If we determine that such information has been inadvertently collected on anyone under the age of 13, we will take the necessary steps to ensure that such information is deleted from our system’s database.
